How to upload documents in case of Reimbursement Claims?

Step 1: Log in to your Pazcare app with your registered mobile number and OTP.

Step 2: On the home page, you will easily find "Raise claim" option.

Step 3: After you raise a claim, you need to upload documents required by the insurer/TPA.

Step 4: The documents include the Claim form, Hospital Bills, Payment Receipts, Doctor consultation, discharge summary, etc.

Step 5: Once you have all the documents ready, upload them by clicking on the "Upload File" option.

(Note: The claim form must be signed by the insured and attested by the hospital.)

Step 6: Once you have uploaded all the documents, click on "Submit"

Step 7: This message confirms that your claim request has been successfully recorded.
